CDS Life Transitions assists people with intellectual and developmental disabilities, veterans, and seniors to live and thrive in their communities by delivering high-quality, personalized life planning and transition support. The CDS Wolf Foundation, named in honor of CDS Monarch founders, Lew & Phyllis Wolf, seeks donations to help support the growth and expansion of CDS Life Transitions' otherwise unfunded community services.
